Before Starting Insulin

  • •Start IV fluids first: rehydrate and correct electrolytes before insulin.
  • •Do not start insulin until serum K is at least 3.5 mmol/L, and continue K supplementation throughout.
  • •Target BG fall: no faster than 2.8 to 4.2 mmol/L per hour (50 to 75 mg/dL per hour).
  • •Monitor K, PO4 and Mg closely (see the Electrolyte Supplementation calculator).
  • •Check BG every 1 to 2 hours.
  • •Serum K not entered.
