Fixed Rate Infusion (FRI) — Dosing

Monitoring Schedule

  • •Blood glucose every 1 hour initially, then every 2 hours when stable
  • •When BG <250 mg/dL: add 5% dextrose to fluids (switch from 0.9% NaCl)
  • •When BG <180 mg/dL: switch to 2.5% dextrose
  • •Target BG decrease: 50–100 mg/dL/hr

Switch to SubQ Criteria

  • •pH >7.3
  • •Bicarbonate approaching normal range
  • •Patient eating voluntarily
  • •Ketonemia resolving
